Tesamorelin and IGF-1 LR3 are often grouped together because both relate to the growth hormone axis. They sit at opposite ends of it, and the literature treats them very differently as a result.
The axis, briefly
Growth hormone-releasing hormone is secreted by the hypothalamus and acts on the pituitary, which releases growth hormone. Growth hormone acts on the liver and peripheral tissues, which produce insulin-like growth factor 1. Much of what is attributed to growth hormone is mediated by IGF-1 downstream.
Tesamorelin: upstream
Tesamorelin is a synthetic analogue of GHRH, stabilised against enzymatic degradation by an N-terminal trans-3-hexenoyl modification. Native GHRH has a very short half-life; the modification is what makes it useful as a research compound.
Because it acts at the pituitary GHRH receptor, the resulting growth hormone release follows the body's own pulsatile pattern rather than producing a sustained elevation. Research has examined this secretion pattern and downstream measures such as circulating IGF-1. Clinical literature has investigated tesamorelin primarily in relation to visceral adipose tissue distribution.
IGF-1 LR3: downstream
IGF-1 LR3 skips the axis entirely. It is a modified recombinant analogue of IGF-1 itself — an arginine substitution at position 3, plus a 13-amino-acid N-terminal extension.
Those modifications reduce affinity for IGF-binding proteins. In native IGF-1, binding proteins sequester the majority of circulating hormone; reduced affinity is associated in studies with a substantially longer circulating half-life and greater free fraction.
Research has examined IGF-1 signalling in cell proliferation, differentiation and anabolic pathway activation, including interactions with the PI3K/Akt and MAPK cascades.
Why the distinction matters
A compound acting upstream is subject to the axis's own regulatory feedback. A compound acting downstream is not. That difference shapes what each is useful for in a research context, and means results from one do not transfer to the other.
